= Terms of Service =<br />
<br />
As a volunteer, you are required to register for clinics online in order to participate. By registering, you help us better coordinate our volunteers, and make accurate transportation and hotel arrangements for our volunteers, as necessary. We also use the site to obtain vital information, such as an emergency contact should the need arise.<br />
<br />
The first time you login to the volunteer site, or anytime the terms of service agreement is changed, you will be given an opportunity to view the terms of service and either disagree, or agree. If you agree, you will be allowed to register for clinics. If you decline, you will be logged off the system. If you decline, your account won't be deleted. Rather, you will be asked to accept the terms of service each time you try to login, and you will only be allowed to login once you accept. Once you accept, you won't be asked again.<br />

The terms of service are subject to change at any time. When they change, we will attempt to notify you by e-mail, and provide a link that allows you to view and accept or reject the new terms. <br />

* Please use a secure password. A combination of random letters or numbers are best, do not use pet names, spouse names, or words that might be found in a dictionary. Do not use passwords that you are using on other sites, such as your e-mail account or banking account. Use a password that is reasonably easy to remember, but hard to guess. If you forget your password, you can use the "Forgot Password?" feature on the site to obtain a new one.<br />

= Accounts =<br />

== Creating a new account ==<br />
<br />
There are 3 steps to creating an account:<br />
<br />
* You will need to sign up. Part of the registration process will prompt for a valid username, e-mail and a password. You do that by clicking on the "Sign up" link on the top of the page.<br />
* Once you are signed up, the system will display a screen telling you an e-mail has been sent to you so that you can activate the account. You must read that e-mail, and click on the link to activate the account. You have seven days to activate the account. If you activate the account, then click on the activation link again, the system will display an error message indicating the account is already active. If you do not activate within seven days, you will need to return to the site and start over.<br />
* Once activated, you can login using the password you entered in step one. When you login the first time, you will be asked for profile information (name, contact info). Once filled in, you can access the rest of the site.<br />
<br />
If not obvious already, you only need to create an account once. Subsequent visits to the site only require you to login.<br />
<br />
You can reset the password at any time on an account by clicking the reset password link when not logged in. You can change the password using the change password link when logged in.<br />

== Bus Payments ==<br />

Starting in February of 2014, we will be accepting online payments for bus travel to and from the clinic.<br />
<br />
Pre-paid seats (those paid for online before the day of use) are given priority. You may pay in cash on the bus the day of the clinic, but seating on the bus will not be guaranteed if you do, as priority will be given to those who pre-pay online.<br />
<br />
When you register for a clinic (or are automatically registered via a recurring clinic registration), and you have selected bus travel, you will be sent an e-mail with instructions on how to pay online. Reminders to pay will be sent weekly until the day of the clinic. <br />

== Hotel Deposits ==<br />
<br />
Starting in August of 2014, we will be accepting online deposits for hotel reservations. We are doing this to try and reduce the number of people who request a hotel but then do not show up or make use of the reservation. We suspect that some of this is due to volunteers who pre-registered for the clinic using the recurring registration feature, and the subsequent year no longer are attending the clinic, or they are attending but not actually planning to use the hotel reservation specified in the recurring registration. Regardless, we expect that the deposit requests will help us better get to the truth of who needs a hotel, and who does not.<br />
<br />
When you register for a clinic (or are automatically registered via a recurring clinic registration), and you have requested hotel accomodations, you will be sent an e-mail with instructions on how to pay a $50 deposit online. Reminders to pay will be sent weekly until the day of the clinic. <br />

If you have registered for a clinic but need to cancel for whatever reason, we asked that you login to the site and delete the registration.<br />

== Registration Changes made By Thousand Smiles on your behalf ==<br />

Thousand Smiles has the ability to modify, delete, or add a registration on your behalf for any upcoming clinic. In the event this happens, you will receive e-mail notification that a change was made.<br />
<br />
= Automatic Registration (Recurring Registration) =<br />
<br />
A ''recurring clinic'' is a clinic that is regularly held at the same time each year, for the same number of dates. Most of Thousand Smiles clinics are recurring, and occur the first Friday in the months of February, May, August, and November. Each of these clinics is two days in length.<br />
<br />
Most volunteers, but not all, return yearly for one or more clinics. If this is you, you will want to register for a recurring clinic.<br />
<br />
== Recurring Clinic Registration ==<br />
<br />
A recurring clinic is not defined for a specific calendar date (e.g., 5/1/2014). Rather it is defined as in the following example "First Friday in the month of February, lasting 2 days". The administrators of the system will predefine recurring clinics in the system, just like all of the other clinics are defined. <br />
<br />
Approximately a month before a recurring clinic, a new clinic is created and made available for registration for the specific date of the recurring clinic. You can register for this clinic by following the directions in Registering for a clinic, above. ic.<br />

== How To Set Up Automatic Clinic Registration ==<br />
<br />
If you would like to automatically be registered for a recurring clinic each year, you must register for the specific recurring clinic. Once you sign up for the recurring clinic, you no longer need to register for the clinic, as the system will automatically register you for the clinic and send you an e-mail to tell you that you've been registered automatically. Once again, this will happen about 1 month before the clinic date.<br />

== What Happens if I am Automatically Registered, But I Can't Attend This Clinic? ==<br />
<br />
Let's say you get an e-mail indicating you were automatically registered, but you are unable to attend. You can sign on and delete the registration as detailed above in Unregistering for a clinic. Your recurring clinic registration will still exist, and you be re-registered around the same time next year. <br />

== How Do I Permanently Remove a Recurring Registration? ==<br />

If you want the system to stop automatically registering you for a recurring clinic, you must delete the recurring clinic registration.<br />

= Deactivating your account =<br />
<br />
You can deactivate your account at any time. Doing so will not cause any recurring registrations you have made to be deleted. However, you will no longer be automatically registered or sent any e-mails until you reactivate the account. <br />

You might want to deactivate your account if you 1) no longer want to participate with Thousand Smiles as a volunteer or 2) you want to take some time off for a period of time.<br />

This person takes the responsibility to provide small toys and/or craft projects for the children in the waiting area from about 9am until lunch time and then again in the afternoon, both on Friday and Saturday at the clinic. There are some crayons and paper kept at the clinic, but we are always in need of more. Coloring books are ok—but there needs to be enough for all the kids to use. Generally, expect to provide for 50 each day. <br />

The kids range in age from birth to teens.<br />

Games such as bingo or lottorea work pretty well. Paper lunch bags can be made into puppets, paper plates can become masks with eyeholes cut into them. Jump ropes are useful too. Face painting and nail polish are popular with the girls. String and beads can become necklaces or bracelets. Board games like Monopoly tend to not work well.<br />

It is helpful if you speak some Spanish, but not necessary. There are usually some teen volunteers to help with the crafts as well, and a person who will give tooth brushing and nutrition lessons sometime during the day.<br />

This job includes putting out trash bags from the warehouse and maintaining some order, sweeping up after crafts are finished, putting away crayons, toys, etc., after the kids are finished. Sometimes the parents will want to participate and that is always ok.<br />

A Rotary club or an independent group generally provides this on Friday and Saturday, because the families are at the clinic almost the whole day. Usually, there is a need for 300 bagged lunches on Friday, and about 200 on Saturday.<br />

The bag lunch should consist of a plastic wrapped sandwich with lunchmeat and cheese, mayo and/or mustard, 2 or 3 plastic wrapped cookies, a drink, and a napkin. The TSF dentists frown upon candy—please don’t include any. Fruit is ok as long as it is something easily cleaned up. Think about healthy meals.<br />
<br />
It is possible to buy the supplies needed in Mexico at Costco. It is advisable to buy them ahead of the day needed. Your group should be at the clinic with the food by 10 am. <br />
<br />
There are tables available to use. You will need plastic tablecloths to cover the table to put your sandwiches together. Generally, lunch is served between 11:30- 12:00 to the patients and their families. Everybody gets a lunch. Occasionally, we get some extra people asking for food from the park. If there is food, please feed them also. Only hungry people ask for food! We want to maintain a friendly relationship with our neighbors.<br />
<br />
Plastic garbage bags are available in the warehouse for trash and cleanup. Please police the area after the lunches are given out. If there is extra food left after lunch, we can see that it is given either to an orphanage or the Alburque where the nuns feed the homeless in Ensenada. They provide beds for our patients who come from out of town—so we try to repay them with food when we are able.<br />

Thank you in advance for your kind offer to prepare lunch for our volunteers. To help your club members have a successful experience, we have tried to standardize our lunches.<br />

We have approximately 100 volunteers at the clinic every Friday and Saturday. Lunch is served from 11:30 until 2pm because with so many people working, they will come in shifts. Please have your members at the clinic in the kitchen by 11am. It is approximately a 2 hour drive to Ensenada from the San Diego border crossing when you use the toll road. Sometimes there is a lot of traffic at the border. <br />
<br />
We recommend leaving early. There is a map for your convenience to our clinic on Avenida Mexico on our website (www.thousandsmiles.org.) You should bring at least 4 or 5 people to set up lunch.<br />
<br />
You will need to bring paper plates, plastic utensils if needed, napkins. We only keep a small supply of paper goods for emergencies. Successful lunches in the past have included: cold cuts, cheese, bread or sandwich rolls, condiments; salads such as potato, green and/or fruit; cookies, chips, soft drinks or fruit juices, and water. We have also had groups bring casseroles, chile, tamales, pizza. We have a full kitchen with a stove, refrigerator; and there is a BBQ grill if you want to barbecue hamburgers and hot dogs. You will need to bring BBQ utensils if you decide to use the grill.<br />
<br />
Please make sure that you have a couple of choices. With 100 volunteers there, we find that the men want a hardy meal while some of the ladies prefer salad and/or fruit. Also, the water and drinks should be plentiful, especially if it’s warm weather. Please plan to stay until 2 p.m. and help with the lunch clean up.<br />
<br />
Should there be leftover food, please know that any extra food you bring will not be wasted. Usually if there are leftovers, we have a volunteer take the food to the rescue mission in Ensenada or a local orphanage. We have a gentlemen’s agreement with the rescue mission. They provide overnight housing for our patients who come from out of town. In return, we donate our extra food.<br />

The Clinic Manager checks in with the following regularly during the clinic and makes sure that things are going to plan. Should problems exist, the Clinic Manager is responsible for bringing the various resources of the clinic together in a way that addresses the problems in the best way possible. In short, this person must make sure each area of the clinic is going well, identify problems and bottlenecks, and make sure responsible parties are notified so that action can be taken in case of problems. Must be aware of the entire clinic operation and the procedures that each department follows (as documented on this wiki). Has overall responsibility for the operation of the clinic.<br />

Charts are delivered to the hub by the triage runner. Each chart is numbered, starting with number 26, the first 25 are saved for known surgery patients. The triage sheet is reviewed. Stickers for each station that triage has requested are placed on the back of the triage page. This page is folded up so that the stickers and number are visible when the charts are in the slots. The number help us keep track of how early in the day the patient arrived. We process the low numbers through the open stations. Once the patients have been through each station the charts are returned to the hub with the patient. The hub reviews the notes in the chart and gives the patient a slip stating when to return. The hub only receives charts, manila in color, for patients that have a history of cranio-maxillo-facial issues. Dental and orthodontic only patients have blue or yellow charts. These charts go directly to the dental desk located to the left of the stairs. If there is a manila chart that should be dental or orthodontic only have a new chart made at the dental desk. These charts are kept at the hub in the "surgery tomorrow" file case at the end of the day or when the patient is completed.<br />

Surgery screening- two runners. This station has 3-4 surgeons each reviewing cases for possible surgery. It is important to have a patient at each surgeon's screening area at all times. Audio screening- one runner. This exam takes between ~30 minutes per patient and can only see one patient at a time, there is no waiting area. The runner needs to keep up on the charts that triage has noted this station as a priority. <br />

If triage has requested dental and or orthodontics and there are no x-rays in the patients chart or they are more than one year old, new x-rays may be needed. Clarification will be needed by the triage Dr. or a dentist.<br />

* Check cables. Despite best efforts, sometimes the power cord to the router on the floor below the computers gets kicked out. The computers should be directly connected to the incoming line from the ISP via a router. There is wireless access in the building, but it is not used for the printer nor for the registration computers.<br />
* If the cables are ok, then perhaps the ISP is down. Contact Bob Chalfa and have him contact the ISP for help.<br />

While computers are down, don't shred the patient registration forms filled out by patients. Use these forms to get or make charts, and then save them in a pile somewhere. Charts will have to manually make routing slips. Once computers are back up, volunteers can enter these patients from the forms, the only difference in all this is that we must manually make routing slips for the patient instead of having them generated during the registration process.<br />

==Chart preparation ==<br />

Currently, all cleft palate patients folders are filed alphabetically and kept in cardboard boxes on top of a counter next to the computers.<br />

On Fridays, volunteers who arrive early help file all folders that are returned to this area that have not been filed from the previous clinic.<br />

The folders from patients that had surgery last clinic are obtained and a list is made of these names. The list is given to the ladies of the Ensenada Rotary working with the patients outside the building. These names are called out throughout the day for follow-up surgical exams. The charts are kept on the counter, in the chart area, in alphabetical order for prompt access. All charts from the surgery patients who do not come to the current clinic are filed with all other charts.<br />

FRIDAY: <br />
Arrive to clinic at 8:00 a.m.<br />
<br />
Remove ADMISSIONS CART from truck. You will need several supplies for both Friday and Saturday.<br />
<br />
SURGERY HUB:<br />
This area will need at least 1-2 runners with at least 1 bilingual apart from the main HUB.<br />
* Arrange a separate stand-up bin for the surgery patient HUB. This area works with the main HUB if (through triage) is determined that other areas such as x-ray, speech, audiology, or dentists need to see the patient. If so, the patient file must pass through surgery, residents, nurses, and pre-op photos before being released to other areas. <br />
* IMPORTANT: take precaution to ensure patient files slated for surgery are given priority and do not get lost in the shuffle. These files must be later gathered and prioritized for Saturday morning surgery admissions.<br />
* IMPORTANT: make sure surgery patients and files flow from the surgeons, to residents for H&P’s, to nurses, and then to pre-op photos. Upon return of file, make a list a list of lab patients (tube patients do not get labs unless he or she is having other surgery). You will need to refer to this list at a later date.<br />

As patient files come from triage it is imperative that each file is given a 1) SmileTrain form, 2) a travel questionnaire, 3) and patient flow sheet for admissions and post-op <br />

The volunteer surgery coordinator will bird-dog all the surgery files and place them in order according to the official surgical schedule as prepared by the surgeons and the OR Director. The hospital's nurse coordinator will advise how many surgical rooms will be available--usually three rooms. <br />

ALL SURGERY PATIENT FILES MUST have H&P’s, pre-admission done, consent forms signed, pre-op photos done, and labs performed (tube patients do not get labs/photos). If not, the file is noted and flagged for Saturday morning completion. <br />
<br />
The files are then transported from the clinic to the hospital by a designated surgery resident. The labs will usually be ready for review the night prior to surgery. Surgeons may elect to cancel some patients at that time.<br />

SATURDAY<br />
<br />
The volunteer surgery coordinator needs a NEXTEL (See Bob Chalfa) for communication between the hospital and clinic for supplies and equipment that may have been overlooked when loading.<br />
<br />
The volunteer surgery coordinator will also need a driver available to go back and forth from hospital and clinic.<br />
<br />
IMPORTANT INSTRUCTIONS FOR EVERYONE concerning the facility and the importance of respecting the hospital requirements of not wearing of scrubs outside the OR. DO NOT arrive to the hospital wearing scrubs if you intend on going in surgery. There is a changing room available. Come in regular clothes, change into your scrubs in the dressing rooms, and when taking breaks or going to lunch...please change back into your street clothing. Nursing staff in pre and post-op can work in street clothing or scrubs.<br />
<br />
BATHROOM LOCKS: If one enters the bathroom and locks the door behind them...the doors must be MANUALLY unlocked otherwise you will exit and the door will shut behind you and be locked. This poses an inconvenience to everyone so please remember to manually unlock (and double check) the door before leaving. <br />

The back ones have special gauges that allow you to fill both duals simultaneously and show you a yellow band with a black line in the middle when the pressure is correct. The fronts should have 90 PSI and will have to be checked with a gauge. There should be a gauge in Bob’s desk, upper LH drawer. You can run a hose from the dental compressors to fill the tires. (The hose is in the equipment room)<br />

The surgical supplies were tracked and controlled from 2002 until mid 2009 a commercial inventory control program called ''WASP MOBILE INVENTORY'' All items are assigned a Thousand Smiles stock number. This number contains seven digits and starts at 1000000. In 2009 we changed to a program called ''VISUAL SUPPLY PRO'' and have started to utilize bar codes to assist in dispensing and reordering items. The dental supplies were added to this system in 2010. For more information on this program please visit their [http://decisionsw.com website]. This program is installed on a server in Bonita. It can be accessed via Windows Remote Desktop where ever there is a computer with internet access. See Bob Chalfa or Syd Logan for access information. We still refer to the WASP program for movement history between 2002 and 2009 whenever necessary.<br />

Items are ordered from a list generated by the computer. This should happen immediately after each clinic. When the ordered items ordered from US suppliers are received in Bonita they are entered into the computerized inventory and a Thousand Smiles barcode label is printed and attached to each item. A case lot label is affixed to the outside of the box on items ordered by the case. These items are preferably tracked as "each" as opposed to "case" or "box" whenever possible, even if they are generally purchased and dispensed in full boxes. The exceptions are items such as gloves, q-tips, ID bracelets and other items that are small. These items are tracked and dispensed by the box. Purchased items are taken to Bob Chalfa's Tijuana office one or two boxes at a time during he 3 month period between clinics. Items purchased in Tijuana are delivered to the Tijuana office. Prior to each clinic the new orders are picked up in Tijuana and taken to the Ensenada warehouse. Each item is placed on the shelf in it's preassigned place usually on Thursday or first thing Friday. <br />

Dental supplies are stored in smaller quantities in a stock room next to the lunch room on the second floor and in the cabinets adjacent to each operatory. These items are restocked as needed from the inventory in the main warehouse.<br />
<br />
We have a surgery supply truck that is stocked with all supplies for the Saturday surgery day. This truck is stocked on Friday after the new order has been put away in the warehouse. Each item's TSF barcode is scanned and in addition a list is made of all items removed from the warehouse and placed in the truck. This list must include quantity, TSF stock number, manufacturer's stock number and description. Accuracy is of utmost importance in this step. Eventually we hope that we will only have to scan the items, but since we are just starting to use the barcodes we feel that the written list should be used at this time. All items removed from inventory and placed in the truck or the dental stock room are treated as if they were used and are deducted from inventory. All items with lot numbers and expiration dates are also tracked by the software. When the individual item barcode is scanned, the scanner will register quantity one. when the case lot label is scanned, the scanner will register the quantity that is contained in a FULL case. Care should be taken to not scan the case lot labels when handling an opened box that may have had items removed. Care must also be taken to only scan the Thousand Smiles label. Items may have 2 or 3 barcode labels. Our labels are unique and easily spotted. If the item does not have our label, do not scan the manufacture's or any other label. Just write down the description and make note that the product did not have a TSF label.<br />

The goal is to have a minimum 6 month supply (2 clinics) of all items in the warehouse along with the stock that is left in the truck and the dental area after the last clinic. Thus we theoretically have on hand 2 to 3 times the quantities we will use of each item at each clinic. Due to the fact that many people have access to the warehouse during a clinic, inventory errors are common, but since we have so much redundancy built into the system, critical shortages have been rare. We do not use any expired items in our clinics with the exception of Methylene Blue which in our program is used only as a skin marker and is not injected.<br />

Up until now most surgical and dental supplies have been ordered on line from several US suppliers. Due to border crossing problems we are presently attempting to purchase more and more items from Mexican suppliers. <br />

All drugs and meds are purchased from the Farmacia del Mar, Ave De Las Arenas #151, Ensenada, BC 02130. Their telephone is (646) 176-2105 and the contacts are Jose Luis Martinez or Laura Oseguera. The order is generated by the computer and placed a couple of weeks before each clinic. TSF has an open account with them. Dental items can be ordered from Dental Azul, in Tijuana. A Rotary club or an independent group generally provides this on Friday and Saturday, because the families are at the clinic almost the whole day. Usually, there is a need for 300 bagged lunches on Friday, and about 200 on Saturday.<br />

The bag lunch should consist of a plastic wrapped sandwich with lunchmeat and cheese, mayo and/or mustard, 2 or 3 plastic wrapped cookies, a drink, and a napkin. The TSF dentists frown upon candy—please don’t include any. Fruit is ok as long as it is something easily cleaned up. Think about healthy meals.<br />
<br />
It is possible to buy the supplies needed in Mexico at Costco. It is advisable to buy them ahead of the day needed. Your group should be at the clinic with the food by 10 am. <br />
<br />
There are tables available to use. You will need plastic tablecloths to cover the table to put your sandwiches together. Generally, lunch is served between 11:30- 12:00 to the patients and their families. Everybody gets a lunch. Occasionally, we get some extra people asking for food from the park. If there is food, please feed them also. Only hungry people ask for food! We want to maintain a friendly relationship with our neighbors.<br />

Plastic garbage bags are available in the warehouse for trash and cleanup. Please police the area after the lunches are given out. If there is extra food left after lunch, we can see that it is given either to an orphanage or the Alburque where the nuns feed the homeless in Ensenada. They provide beds for our patients who come from out of town—so we try to repay them with food when we are able.<br />

Thank you in advance for your kind offer to prepare lunch for our volunteers. To help your club members have a successful experience, we have tried to standardize our lunches.<br />

We have approximately 100 volunteers at the clinic every Friday and Saturday. Lunch is served from 11:30 until 2pm because with so many people working, they will come in shifts. Please have your members at the clinic in the kitchen by 11am. It is approximately a 2 hour drive to Ensenada from the San Diego border crossing when you use the toll road. Sometimes there is a lot of traffic at the border. <br />

We recommend leaving early. There is a map for your convenience to our clinic on Avenida Mexico on our website (www.thousandsmiles.org.) You should bring at least 4 or 5 people to set up lunch.<br />

You will need to bring paper plates, plastic utensils if needed, napkins. We only keep a small supply of paper goods for emergencies. * Turn on the main propane tank.<br />
* Make sure the 5 gal spare tank is over ½ full<br />
* Light the hot water heater<br />
* Turn on the recurculating pump<br />
* Check the filter indicators on the compressors, and if in the green plug in both compressors and turn one on.<br />
* Check the outlet hoses on the 2 vacuum pumps. They should be in the drain tubes to the left of the pumps.<br />
* If all is ok, plug in and start the 2 vacuum pumps<br />
* At the end of the day on Saturday, do just the opposite and shut everything down. <br />
* Check again Sunday morning to make sure everything is shut down and the equipment room door locked.Surgical and dental supplies are stored in the main warehouse in the rear of the building adjacent and to the south of the clinic.<br />
